Prepusiyumda balanitis xerotica obliterans varlığının araştırılması

Abstract

Bu klinik çalışmada çeşitli endikasyonlarla yapılan prepusiyum eksizyonu işlemlerinde alınan doku parçalarında balanitis xerotica obliterans (BXO) varlığının araştırılması amaçlanmıştır. Bu amaçla Uludağ Üniversitesi Tıp Fakültesi, Çocuk Cerrahisi Anabilim Dalı'nda Nisan 2003 ile Mayıs 2005 tarihleri arasında değişik sebeplerle opere edilen hastalardan eksize edilen sünnet derisi materyalleri, Uludağ Üniversitesi Tıp Fakültesi, Patoloji Anabilim Dalı'nda incelenmiştir. Toplam 215 çocuk çalışmaya dahil edilmiştir. Hastaların yaş ortalaması 4.9 yıl olup, 1 ay ile 15 yıl arasında dağılmaktaydı. Olguların % 26.5'ini (n=57) 0 - 1 yaş arası çocuklar, % 29.8'ini (n=64) 1 - 5 yaş arası çocuklar, % 36.7'sini (n=66) ise 6 -10 yaş arası çocuklar ve % 7'sini (n=15) ise 11 -15 yaş arası çocuklar oluşturmakta idi. Sünnet endikasyonlarına göre; fimozis tanısı ile sünnet edilen 56 (% 26) hastanın 2'sinde (% 3.6), sünnet istemi olan 124 (% 57.7) hastanın 2'sinde (%1.6), üriner sistem sorunları sebebi ile sünnet edilen 13 (% 6) hastanın 1'inde (%1.2) histopatolojik olarak BXO düşündüren bulgular saptanmıştır. Fimozis ve üriner sistem sorunları sebebi ile sünnet edilen 8 (% 3.7), hipospadiaslı 14 (%6.5) hastanın hiçbirinde histopatolojik olarak BXO saptanmamıştır. Sonuç olarak BXO hastalığına bağlı fibrotik değişiklikler 2 yaş civarında başlamaktadır. Sünnetin küçük yaşlarda uygulanan bir cerrahi işlem olması nedeniyle bu hastalık ülkemizde şu ana kadar sık olarak görülmemektedir. BXO tanısı almış hastalar titiz bir şekilde uzun süreli takip edilmelidir.

In this clinical study we aimed to investigate the presence of balanitis xerotica obliterans (BXO) in the tissues obtained from prepucium excision materials for different clinical conditions. For this purpose, prepucium materials excised from operated patients for different causes in University of Uludag, Faculty of Medicine, Department of Pediatric Surgery between April 2003 and May 2005 were examined in the laboratories of pathology department. Totally 215 children were examined in this study. Their mean age was 4.9 years, ranging between 1 month and 15 years. When the cases divided into groups according to the ages, 26.5 % of cases (n=57) were 0-1 year old and 29.8 % of the cases (n=64) were 1-5 years old and 36.7 % of the cases (n=66) were 6 -10 years old and .7 % of the cases (n=15) were 11-15 years old. When the patients were grouped according to the circumcision indications, findings that suggested BXO as histopathologically were determined in 2 patients (3.6 %) of 56 patients (26 %) circumcised for phimosis, 2 of 124 patients (1.6 %) circumcised for social and religious pourposis and 1 of 13 patients (1.2 %) circumcised for urinary system problems. BXO were not determined in the 8 patients (3. 7 %) circumcised for phimosis and urinary system problems and 14 patients (6.5 %) with hypospadias. Finally, fibrotic changes related with BXO disease begin around 2 years. As the circumcision is performed in childhood, this condition is rare in our country. Patients diagnosed as BXO have to be followed-up fastidiously for a long time.
