Ptoziste müllerektominin etkinliği ve gözyaşı fonksiyon testlerine etkisi

Abstract

Amaç: Üst gözkapağı ptozisinin tedavisinde konjonktiva-Müller kası rezeksiyonu yönteminin etkinliğini değerlendirmek ve bu tedavi sonucunda gözyaşı fonksiyonlarında değişiklik olup olmadığını araştırmaktır. Gereç ve Yöntem: Bu çalışma üst gözkapağı ptozisi bulunan 27 hastanın (17 kadın, 10 erkek; ortalama yaş: 32,6 yıl) 28 gözünü içerdi. Tüm hastaların fenilefrin testi pozitifti ve levator fonksiyonu iyi derecedeydi. Ptozis miktarı ve fenilefrin testine yanıta göre, 5-11 mm arasında konjonktiva-Müller kası eksize edildi. Toplam 9 gözün preoperatif ve postoperatif yapılan oküler yüzey ve gözyaşı fonksiyon testleri karşılaştırıldı. Postoperatif ortalama izlem süresi 9.4 aydı (1 - 23 ay). Sonuçlar: Blefaroptozis 15 hastada edinsel, 12 hastada doğumsaldı. Ptozis miktarı 1.0 ile 5.5 mm arasında değişmekteydi (ortalama: 2.18 mm). Ameliyattan sonra iki gözkapağı arasında 23 hastada (% 85) 0-0.5 mm, 2 hastada (% 7,5) 1 mm ve 2 hastada (% 7,5) 1,5-2 mm farkla simetri elde edildi. Sekiz mm veya üstünde rezeksiyon yapılan gözlerde gözyaşı film stabilitesinde bozulma eğilimi saptandı. Dört hastada (%15) erken postoperatif dönemde geçici oküler yüzey komplikasyonları gelişti. Tartışma: Konjonktiva-Müller kası rezeksiyonu, üst gözkapağı ptozisinde levator-aponevroz cerrahisine kıyasla teknik olarak daha kolay ve daha az travmatik bir yöntemdir. Bu yöntemle hastaların çoğunda, öngörülen ameliyat sonuçları elde edilebilir. Konjonktiva-Müller kası rezeksiyonu, uygun hastalarda levator cerrahisine iyi bir alternatif olabilir. Bu ameliyat yöntemi gözyaşı film stabilitesini olumsuz etkileyebilir.

Purpose: The first aim of this study was to evaluate the efficacy of the method of Müller's muscle-conjunctival resection in the treatment of upper eyelid ptosis and the second aim was to investigate whether the tear function tests alter by this surgical method or not. Materials and Methods: This study included 28 eyelids of the 27 patients (17 female, 10 male; mean age: 32,6 years) with upper eyelid ptosis. All patients had a positive phenylephrine test and good levator function. Based on the amount of the ptosis and response to the phenylephrine test, 5 to 11 mm conjunctiva-Müller's muscle was resected. The tear function and ocular surface test results which were obtained preoperatively and postoperatively, were compaired in a total of 9 eyes. The mean postoperative follow-up time was 9,4 months (1-23 months). Results: Blepharoptosis was acquired in 15 patients, congenital in 12 patients. The amount of ptosis ranged from 1.0 mm to 5.5 mm (mean, 2.8 mm). After surgery, the two eyelid levels were symmetrical within 0.0-0.5 mm in 23 patients (85 %), within 1mm in 2 patient (7,5%) and 1-2.0 mm in 2 patients (7,5 %). We found a deterioration tendency in the tear film stability in patients who underwent 8 mm or more the conjunctiva - Müller muscle resection. In 4 patients (%15), transient ocular surface complications developed in the early postoperative period. discussion: Muller's muscle-conjunctival resection is technically easier and less traumatic method compared to the levator-aponeurosis surgery in upper eyelid ptosis. By this method, predicted surgical results can be obtained in most patients. Muller's muscle-conjunctival resection may be a good alternative to the levator surgery in appropriate patients. This surgical method may influence the tear film stability adversely.
