Asetabulum kırığı olan hastalarda primer total kalça protezi uygulaması sonrası radyolojik ve fonksiyonel sonuçlar

Abstract

Asetabulum kırıkları sonrası erken Total Kalça Protezi (TKP) uygulamasının fonksiyonel ve klinik sonuçlarının değerlendirilmesi amaçlandı. Ocak 2008-Ekim 2013 tarihleri arasında Uludağ Üniversitesi Ortopedi ve Travmatoloji Anabilim Dalı Kliniğine başvurmuş, asetabulum kırığı tanısıyla erken total kalça protezi uygulanan 17 hastanın klinik ve radyolojik sonuçları değerlendirildi. 17 hastanın, 14'ü erkek, 3'ü kadın olmak üzere, ortalama yaşları 52 (29 – 80) dir. Hastaların ortalama takip süresi 31,64 aydır (7 ay – 70 ay). Hastaların travmaları ile operasyonları arasındaki ortalama süre 13 gündür (2 gün-21 gün). Hastaların fonksiyonel durumları Harris kalça skoru ile değerlendirilmiştir. Ortalama harris kalça skoru 89,64 (70-100) olarak bulunmuştur. 13 (%76) hastada ise mükemel ve iyi olarak bulunmuştur. Travmadan önce çalışan 10 hastanın 7 (%70) si aynı işe geri dönmüştür. Ortalama işe dönüş süresi ise 7,2ay (1,5 ay-24 ay) dır. Komplikasyon olarak kısalık, enfeksiyon, dislokasyon ve heterotopik ossifikasyon görüldü. Enfeksiyon ve dislokasyon nedeniyle iki hastaya revizyon uygulanmıştır. 9 (%52,9) hastada Brooker sınıflamasına göre heterotopik ossifikasyon tespit edilmiştir. Asetabulum kırıklarından sonra doğru endikasyon ve doğru hasta seçimi ile yapılan erken TKP ile ağrısız ve fonksiyonel bir kalça eklemi elde edilirken, hastanın erken mobilizasyonunu, erken işe dönüşü, reoperasyon riskinde azalma gibi fonksiyonları olduğunu düşünmekteyiz.

Functional and clinical assessment of early Total Hip Prosthesis (THP) application after acetabular fractures was aimed. Clinical and radiological results belonging to 17 patients who applied to Uludağ University, Orthopedics and Traumatology Department Clinic and who were diagnosed with acetabular fracture and were applied early total hip prosthesis between January 2008-October 2013 were assessed. Out of total 17 patients; 14 patients were male, 3 patients were female, and the average age was 52 (29-80). Average follow-up period of the patients was 31,64 months (7 months-70 months). The duration between the traumas and operations of the patients was 13 days (2 days-21 days). The functional status of the patients has been assessed with Harris Hip Score. The average Harris Hip Score has been found as 89,64 (70-100). It has been found as perfect and good in 13 patients (%76). Out of 10 patients who were working before the trauma, 7 patients (70%) went back to the same work. Average back-to-work duration was 7,2 months (1,5 months-24 months). As complications, shortness, infection, dislocation and heterotopic ossification were observed. 2 patients were applied revision due to infection and dislocation. According to Brooker Classification, Heterotopic Ossification (=HO) was determind in 9 patients (52,9%). After acetabular fractures, with the right patient and indication selection, painless and functional hip joint was obtained with the early THP application. It is considered that this application has important functions in early mobilization, early return to work opportunities and in decreasing re-operation risks.
